Mentoring NP- Pharmd Learners in a Sustainable Interprofessional Dedicated Teaching Clinic: Everyone Teaches, Everyone Learns
The 2025 ADA Standards of Care state that, “People with diabetes can benefit from a coordinated interprofessional team that may include but is not limited to diabetes care and education specialists, primary care and subspecialty clinicians, nurses, registered dietitian nutritionists, exercise specialists, pharmacists, dentists, podiatrists, and behavioral health professionals.” The rising prevalence of diabetes coupled with the predicted shortfall of endocrinologists increases the need to develop new diabetes care models. Collaboration between pharmacists and nurse practitioners (NPs) in diabetes clinics represents an effective approach to meet care gaps and improve patient outcomes. Training students in a real world interprofessional setting is the best way to promote future team based practice and cultivate a supportive learning environment that actively promotes clinical excellence. The presentation explores the synergistic roles of pharmacists and NPs in managing diabetes, emphasizing their complementary expertise in medication management, patient education, and chronic disease management in a sustainable clinic. This interprofessional model intentionally pairs pharmacist and NP learners to facilitate growth of integrated teams in other settings. Case studies will illustrate how integrated care enhances glycemic control, reduces complications, and improves patient satisfaction. Attendees will gain actionable insights to implement collaborative care and integrated teaching models to provide complex diabetes care and advance clinical outcomes.
